چکیده

OBJECTIVE To assess the rate of successful antiepileptic drug (AED) discontinuation, prognostic factors and proper time of AED withdrawal after surgery for medial temporal lobe epilepsy (MTLE). METHODS We reviewed 171 consecutive patients who underwent resective surgery for MTLE. All patients were followed up for more than two postoperative years. AEDs were slowly tapered with an individualized schedule for each patient. Outcome status was determined from medical records and telephone interviews. RESULTS 41.2% of patients experienced no seizure recurrence. 34.5% discontinued medication without seizure recurrence for more than 2 years at final assessment. Multivariate analysis revealed that an age greater than 30 years at surgery and postoperative AED reduction before 10 months increased the risk of recurrence [hazard ratio (HR) 2.1, 95% confidence interval (CI) 1.1-3.9 and HR 2.5, CI 1.1-5.8]. CONCLUSIONS Resective surgery for MTLE brings seizure remission without AED to one-third of patients. Postoperative AED tapering is recommended after at least 10 months. Younger age at surgery is a good predictive factor of remission after MTLE surgery.

The Determiners of Temporal Lobe Epilepsy Surgery Outcome in a Developing Country

Background: Epilepsy surgery has become a successful treatment option for patients with drug-resistant epilepsies. Objectives: This study aimed to assess temporal lobe epilepsy (TLE) surgery outcome and its association with epilepsy duration, the presence of pre-surgical generalized tonic-clonic seizure (GTCS), and brain MRI findings. Materials & Methods: This retrospective study was perfo...

Anterior temporal lobectomy versus selective amygdalohippocampectomy in patients with mesial temporal lobe epilepsy Lobectomia temporal anterior versus amigdalohipocampectomia seletiva para epilepsia de lobo temporal mesial

It is estimated that one third of patients with seizures have medically intractable epilepsy (MIE) – defined as failure of two antiepileptic medications given at appropriate doses1,2. Temporal lobe epilepsy (TLE) is the most common form of MIE. The most frequent pathologic substrate related to this condition is sclerosis and atrophy of the hippocampus – disease named mesial temporal sclerosis (...

Early antiepileptic drug reduction following anterior temporal lobectomy for medically intractable complex partial epilepsy

PURPOSE To determine the safety, in our practice, of allowing patient preference to influence the timing of antiepileptic drug (AED) reduction, once they became seizure-free after anterior temporal lobectomy (ATL). METHODS Thirty patients underwent anterior temporal lobectomy for medically intractable complex partial epilepsy at Loma Linda University Medical Center between December 1st 1991 a...

Antiepileptic drugs and relapse after epilepsy surgery.

PURPOSE To evaluate whether the postoperative, antiepileptic drug (AED) regimen influences seizure recurrence after anterior temporal lobectomy when considering the putative mechanism of action and possible neuroprotective effects. METHODS This was a retrospective study. Patients who had an anterior temporal lobectomy for refractory epilepsy, whose preoperative MRI indicated mesial temporal s...

Relationship of number of seizures recorded on video-EEG to surgical outcome in refractory medial temporal lobe epilepsy.

INTRODUCTION Surgical planning for refractory medial temporal lobe epilepsy (rMTLE) relies on seizure localization by ictal electroencephalography (EEG). Multiple factors impact the number of seizures recorded. We evaluated whether seizure freedom correlated to the number of seizures recorded, and the related factors. METHODS We collected data for 32 patients with rMTLE who underwent anterior...
